so i went for a long run yesterday, 11 miles. . i have been running a long run once a week (10 last week), and 1-2 short (3.2 mile) runs once a week. my knee had never hurt at all prior to this. the pain is on the outside of my knee where the joints connect the lower leg and upper leg. it hurts when i bend my leg from straight to about 60 degrees, but then doesn't hurt as i make my shin perpendicular with my quads. it is tender if i push it as well. from looking at knee diagrams, it looks like it could be meniscus? maybe ITB, but i think ITB is farther back. i can put weight on it, but it just hurts to bend.

i've had those same pains.. it was my IT band.. increased distance and shoes could be the cause of the pain..

That's a big jump running 3.2 miles to 10 or 11 miles... I think you workouts should progress to the distance you want to run at the end of the week..

My daughter just went through a knee exam, X-ray, MRI and another exam. Looking at the MRI with orthopedic and listening to him explain all the parts of the knee confirms my believe you need to see a sports orthopedic. There are just too many possibilities.

That said, have you tried RICE (Rest, Ice, Compress, Elevate)? Did the pain go away the following day. If the pain went away, reduce running, ice after exercise and take some Motrin. If the pain does not go away, schedule an appointment.

(FYI, my daughter's first exam indicated might be torn miniscus. MRI proved negative. Suspect one of the fatty pads in knee is inflamed. Injection of cortizone (man, I could not watch THAT!) to reduce the inflamation)

Also, if you can do so without pain, make sure to stretch a lot. Work on those IT band stretches, just in case.

Foam roller - hurts like a motherf*er, but it helps a lot.

with regards to the 3.2 to 11 miles, i worked up to the long run over the course of a couple months, doing 5, or 6 miles then up. . the 3.2 i do once/twice a week is more of a tempo run.

i think it is probably more just joint soreness. . it feels better now (just later in the day and i iced this morning), i can bend it w/o too much uncomfort. . i think it may just have been too much too soon, i have ran the same amount this month (so far) as i did last month (for the whole month), which does not follow 10%, i am a bad citizen

Foam roller - it works   but you have to be religious about using it.

I ordered mine online from performbetter.com

I bought the PB ELite Molded Foam Roller.   It's black and much harder than the traditional white ones so it lasts longer, doesn't get all soggy or mushed down in the middle with use.

What size should I get if I'm using it for the ITB band (maybe that was redundant)?

I don't think there are sizes. Just the basic bolster.

I've been dealing with this same thing for a while now.  Foam roller is good, you might Google "Reynolds special" exercise.  It's been a big help for me, that and stretching religiously.  I had been pain free for the last few months until the Portland Marathon and the dang thing kicked in at about 6 miles.  Had to walk the rest of the marathon....in the rain.  Oh my, I'll be going to see the doc or therapist before I try that again.

Target used to carry them, they might still if you want some instant gratification.  Get the longest one you can.  I got a med length one and I have times I'm afraid I'll go off the side of it.

Ok this sounds like the location where the ITB crosses over the lateral tubercle of the tibial plateau.... What surfaces and gradients are you running on?

ITB syndrome often occurs when the knee has repetitive movements causing the ITB to flick over the lateral epicondyle of the femur at around 30 degrees of flexion- ie especially in downhill running. Im guessing that at least 30% of your run is on a downhill slope of around 1-2%.

If Im correct, try walking down a flight of stairs- if this reproduces your pain on the lateral side of your knee, then you can should try a couple of things:

1: wurkitgurl is spot on. Massage and Stretching to the ITB to lengthen it out (can be done with the rollers or by a massage therapist/physio- both hurt like a mofo!)

2:Avoid the aggravating factors (ie change where you are running- try running at your local running track instead)

Good luck!
