Subject: Inside Knee Pain - Cannot run on it

This is a problem - I am 4 weeks out from Marine Corps Marathon (and yes, I am going to see about transferring my bib or deferring until next year, depending on the time limits). I have had some ITB issues on my right side, but stretching and rolling, etc have seemed to take care of that pretty well. Last Friday (the 19th) I went for my 18-miler. Halfway through, my LEFT knee started to hurt and then very quickly started to be VERY painful. I had to hobble/walk the majority of the way back. Didn't run on it again until Thursday - only ran for about 40 minutes. Ran again on Friday for about 50 minutes, but it started to hurt again, and then it hurt mildly for the rest of the evening - since then, it has hurt slightly when I walk (in flat shoes). I got a new pair of running shoes, thinking they were just worn out (had about 250 miles on them) and started out fr 14 miles today - I was able to do 3. With a knee brace, even. It still hurt so much that I had to stop. It's on the INSIDE of my left knee, not the front (patellar tendon) or the side (ITB), the inside edge of my knee. I was running on a flat trail, though there were lots of small rocks in the ground. I didn't trip or fall or anything. It just started to hurt all of a sudden.

I am going to try to see a specialist as soon as humanly possible, but I have an HMO, so it'll take a week to get a referral. I am obviously going to consult a medical professional for a real diagnosis, but I am just curious if other people have experienced this and what it ended up being/what ended up happening. I have not had ANY problems in my left knee before.

To clarify - during the 18 miler, I was running on the dirt trail with the small rocks. Today, I was running on asphalt. I am wondering if running on the rocks caused my left ankle to roll inward, putting pressure on the knee - enough to damage it in some way, that it would continue to hurt while running on a flat, even surface.

That stinks and I can sympathize. I've been battling ITBS on and off this year (did IMCDA in June) and just finished my last 20 miler in prep for Detroit Marathon coming up in three weeks. Started to feel a bit "tweeky" during the last couple of miles, hopefully a smart/easy taper will allow things to thaw out in time....

DISCLAIMER: I'm not an ortho or a PT, just a chronic knee pain sufferer.

Based on experience, I'd be suspicious of synovial plica syndrome. Plenty of info out there via google and running can really aggrevate it, I've been there..... Do you have an area that is really tender to the touch? It could also be a meniscus tear, but it doesn't sound like you've suffered any trauma recently? I'd start icing 2 or 3X per day.

Quite a few muscles coming down from the hip area attach there on the inside of the knee.  Probe the muscles starting a couple inches above your knee (gracilus, adductor, vastus medialus).  You'll likely find a knot or two that is quite sensitive to pressure.   

 You might want to visit http://www.julstro.com/ .  The ebook "Pain-Free Living" at $21.95 is a worthwhile investment.  Don't buy "Pain-Free Triathlete".  It is 3-4 years older than Pain-Free Living.   I've had problems on both the inside and outside of my knee and am currently using that book as a reference.
