Vision aerobar malfunction?!!?!?!?

So I'm finishing up a 50 miler this past weekend and I feel that my right aerobar is a bit loose.  I think "no big deal, I'll just tighten it up when I get home".  Unfortunately, I didn't quite get home before the aerobar actually slipped off of its fitting, swing down into my front wheel, and got jammed up against the fork.  even though the aerobar felt a bit loose, there was no reason for me to believe that this would happen.  Lucky for me, I was cooling down on a fairly crowded beach path, but evenso, I went up and over the front wheel and came down on my head and shoulder and slid on my right side for a bit.  Other than some road rash, a sprained wrist, and a bruised ego, I am mostly okay.  Needless to say, my 4 month old P2C is a mess: bent spokes, broken shifter, destroyed cable, tweaked caliper, and a potentially busted fork.

Anyway, has anyone else ever heard of this happening before?  Until this incident, I thought that the aerobars were one piece (besides the arm wrests and the clip on part).  So the bike is now back with the LBS where I bought it and apparently they're going to call up cervelo tomorrow and send some pics.  I'm hoping that this whole thing is taken care of since there is no reason why aerobars on my bike should be falling off.  I guess I'm pretty lucky that this incident didn't happen on the busy road that I had been riding on just minutes earlier where I would have been going much faster.  It freaks me out to think about it.
