Just got back from my MR Athrogram a few minutes ago. 13 months after my SLAP repair, I just found out I re-tore my labrum again when falling off my triathlon bike while training for the half-ironman.


I have a known buford complex and a history of failed thermal shrinkage surgery in 2000 on the same shoulder.

What are my options?

I really busted my butt throughout my rehab with pt, and continued taking good care of my shoulder until I got injured. I stretched every day, and competed in triathlons after the rehab.

Triathlons are a big part of my life.

I am devastated. What is the next step to get me back on track? My arm was perfect again, before I fell off my bike.

I'm 29 years old .

I was trying to become familiar with clipless pedals on my new triathlon bike when stopped and fell to my right side, bracing my fall with my right arm.

What options do I have to return to triathlon training and have a stable shoulder?

I have another appointment on Tuesday with Orthopedic Surgeon Dr. Stephen Kottmeier at Stony Brook University Hospital in New York. Dr. Kottmeier specializes in sports medicine among other things, and was voted one of New York's best doctors.

Im really sorry to hear about your fall, and injury Sounds both painful and frustrating. I'm totally unqualified as to the rehab of this injury though... I think its best you talk with your sports doc about it. I hope you are able to get back in the sport as quickly as possible.

Just to make you feel a little better (and give you hope)...I have a tendency to have my shoulders partially dislocate as well.  I went ahead and got my left labrum corrected a couple years ago...went to Hawaii back in Sept. and had my right should come out...I have postponed surgery on that shoulder until after IMAZ in Nov.  The only problem I have is when swimming, but I over rotate in order to keep my shoulder in a more forward position (up and back position triggers the subluxsation (sp?)  So there is hope for you to continue your tri-life with this issue.  My left shoulder hasn't given me problems since the surgery.  PT was fun (not!) but worth it.  Good luck to you!

I had a SLAP repair on my left shoulder 10/06, SLAP repair on right shoulder 3/07, thermal capsule shrinkage on right shoulder 12/2000. This current injury is to my right shoulder. It felt perfect before the fall.

Are you planning to complete IMAZ with this injury as well?

The only advise I have is to keep your options open and be as honest and upfront as you can with your Ortho next week. (About your training and your race goals)

Make a list of questions to ask him and talk about your options.

I just went through a Labrum Repair last December (SLAP and Bankart Repair). My ortho cleared me for full training 11 weeks post-surgery and I was able to do my HIM about 4 months Post-Op.

I was really clear with him about what my goals were but I was also open to the idea of DNS'ing early season races including my HIM so that I could be healthy for my IM in June.

Sounds like you have been through the process before so you already know what to do.

I dislocated my left shoulder twice, once in 2002 playing basketball and once last january sking (I'm a 25 yr old male). I did the whole MRI thing and also had a torn labrum. The orthopedic surgeon was pushing for the surgery but I opted for physical therapy instead. Id say within 2 months I was lifting and swimming and havent had any problems since. My best advice is learn to fall. Not being a smart a$$, but have had several accidents since and no problems.

Look at it this way. If you get the surgery you are guarenteed to be out for most of the season. If you go the PT route you can be back on the bike in 3 weeks and might be able to squeeze in some du's/tri's in August.

Having the surgery wont guarentee that it wont pop out again. I reccomend making the theraband your new best friend and go the PT route. That decision worked great for me.

I'm sitting in front of my computer right now, and using my left arm, with minimal pressure, I can push the back of my humerus forward, and it pops forward as if nothing is holding it in place anymore. I am frightened.

I'm looking for a shoulder specialist in the tri-state area who has seen my situation before and help me get back to triathlons with a stable shoulder, pain free, and range of motion.

I go over the MRI results with a doc on Tuesday.

I have a feeling I will need a surgery called an open capsular shift.

Sorry to hear, but you and I had talked about the possibilty. Just go for it and get it fixed. Can't live in the past. It's been 2 weeks since my repeat Labrium surgery and I just smile and look forward. You are only 29 which is great. I'm 44 so the healing and PT take longer. However, I will be right back out there for IMAZ in Nov. Walked 4 miles with my arm in a sling this morning and have my computrainer set up next to a shelf that I can hang onto with the left side of my body to keep my legs in shape.

If its moving that bad there is no way the swim is going to be anything but trouble (in my opinion), so if you don't go for it now and get it fixed you could damage it further, dislocate it at the wrong time (Middle of the 1/2 swim and get run over) or just waste time by waiting for something that NEEDS to be fixed.

By the way, the first time I tore mine it didn't even show up on the MRI so they didn't know what they would find inside. This last time it was visible for good reason.Just like yours, if they can see it........

I think I need more than a labral repair because my shoulder subluxated on the apprehension test performed by the doc.


SLAP repair or open capsular shift? Was this last surgery you had opened or arthroscopic? My last was a type II slap repair along with an arthroscopic capsular shift incorporating my middle glenohumeral ligament (considering I have buford complex, and that was the only viable ligament).

I also had the Bankhart repair done due to the shredded tendons in and around the socket.  Lots of cool pics of the shoulder if you want to see...!  It looks like a womans...oh wait...this is a public forum...nevermind.

The one 7 years ago and the one 2 weeks ago were both arthroscopic and they just reattached the Labrium. Don't know what the procedure is called but both times it involved putting tiny anchor screws in the bone to tie the labrium back up. For 7 years after the first one my shoulder was tight and felt great. Hopefully it will be the same this time.

I let the Doctors assistant know about what I did she said if you lifted above your limit you run the risk of re injury! so let us hope and pray you didn’t damage it ! I feel a pulling sensation light but still its not fun I don’t know if this is a pain I should be worried about or if I’m just overthinking it ! My hand tingles off and on but its possible when I hurt it the next day I went into PT and lifted threw the pain and the 2nd session at PT that week about two days latter I told my PT…. I was sore and needed to take it easy ! so PT was not happy about my pore choice but said you should be fine since at that time I thought I was getting better turns out ive been not doing much with it but ice and a pulley system and use it twice a day for range of motion so I don’t get stiff and such but my question is (A) should I discontinue my exercises for a week ?I canceled this week from PT to see if things get better plus I cant afford for me to pay big bucks for going “easy on it ” if you know what I mean ! To be safe or considering I don’t have full range of motion yet although I’m almost there just have a hard time reaching behind my back but I can do it! and (B) at what point should I be concerned with this set back? Like I said I hurt it last mon ??? going back to my DR, I see my surgeon on 2/12/18 for my 3rd month follow up vist they said whatever happened happened and its to early to tell what happened……. apparently its also too early to get a mri done on it so I pray and hope if I just lay low this week with ice and try not to move my arm around much considering going on week 2 still not much reduction in pain although it doesn’t feel like theres anything moving anymore however now its more of a burning sensation errrr ive come so far I really hope I didn’t screw my self what a costly mistake but maybe ill make it out of this mess !


That's my story so its been like ten days now and the pain is a little better mainly in the morning and at night right before bed so its not looking to good! anyone here have a revision surgery if so what the process and how did you coupe! it seems like that the only way to truly find out if its re torn is to go in the shoulder a 2nd time since the MRI s can miss tears micro tears!

I feel your pain brothers at lest we have each other to chat haa misery loves company !

I had a really bad grade 4 slap tear a few years ago. It was a chronic injury that just got worse after years of playing rugby. I am 34 now but was 32 at the time. I was borderline age for a traditional SLAP repair vs. Bicep tenodesis. My surgeon, in the the Indianapolis area recommended leaning toward the bicep tenodesis (reattach the bicep tendon in the upper humorous instead). Basically there is better blood flow and it heals much faster. I was able to resume playing rugby in about 8 months. I was in the pool at about 10-12 weeks. I regained all my strength, actually a little more than before I had surgery; Since I had a chronic tear that was causing chronic weakness. I played 1 more full year of rugby and then decided to switch focus to a more sustainable sport. Now, I'm here. I had great success with my surgery. I think the initial 6 weeks was absolutely awful. However, when the swelling went down my rehab got exponentially better and I fell back in love with the fitness I got from swimming. I probably wouldn't be participating in tri at all if it wasn't for that injury because I hadn't swam as a workout in years and hadnt done it consistently since middle school swim team. You won't need 8 months like I did for rugby, but you will need at least 4-6 months to get back to reasonable swimming shape. Good luck. Let me know if you have anymore questions.

Sad to hear your situation. I'm curious if how are you doing right now? Since it has been years ago. I had the same problem but it's my feet. Luckily, I'm fully recovered and starting my triathlon journey since last year. Hoping you are well now
