I usually just tuck it in my shorts so it rests on my thigh.  Most of my shorts are fairly snug with the leg gripper cuffs.  I've had it slide arounds somewhat, but it's never fallen out.

in my hand because i am either texting or changing what song I am listening to, while wearing a bunch of Ironman branded gear (I haven't done one), no helmet, on the sidewalk or on the road going against traffic during rush hour.

I have a small phone...so it fits in the gel pocket of my tri shorts.

The fact that my phone FITS in there is the number one reason I refuse to actually upgrade to a better Blackberry......
