Pre-race routine:

Small oatmeal with greek yogurt, small coffee w/ milk

Comments:

Really good feeling swim; relaxed, long even strokes, no shoulder pain; could have gone faster but intentionally held back to protect shoulder. INNOVATION: Put prescription sunglasses inside shorts so I'd have them as soon as I exited water -- worked very well.

Intentionally kept cadence fairly high to try to save legs; not sure it worked; average speed was actually somewhat higher than I planned but it was comfortable;
Dropped extra water bottle :-(
Potato wedges pretty good but hard to get down -- kind of dry

First 2 1/2 miles felt really fine and was at a good pace; At 2 1/2 miles, right quad cramped -- stopped to massage -- and when I started running again, my heart race skyrocketed to over 180. I stopped running and walked to mile 3 and stood under fire hydrant spray. That plus ice water and Hammer gel helped a lot and I mostly jogged to mile 4. Took several frozen towels and water and had 2nd fuel belt bottle of HEED. Walked through turn and when I started running, left calf and right hamstring cramped. Mostly walked with short periods of jog to mile 6. Decided to try running final stretch but my legs had different ideas -- both legs cramped in calves, hamstrings and quads -- I literally toppled over. I sat and tried to massage them out, got up and hobbled in serious agony to finish.
What would you do differently?:

More electrolytes, better heat training

Warm down:

Cooled off in EMT tent and then cold mist tent; a few bottles of water; got sick in the car on the way home :-( This is fun?!

What limited your ability to perform faster:

Event comments:

Despite suffering heat exhaustion, I am satisfied with my performance. I prepared as best as I was able under the circumstances (work, life), had very good swim, bike, and 1/3 of the run -- the heat did me in.
