Comments:

The first half of the swim, until the turn around buoys, was bad. I started out in the back of the pack at the start, which caused me to get stuck behind a lot of the slower swimmers. I was constantly catching feet and could get around as their were swimmers to the left and right.
Once I got to the turn around buoys I swam a little wide to get into some open water and around the slower swimmers and was finally able to start passing and swimming at my pace.
What would you do differently?:

I'm not sure if the organizers changed the wave start from M25-29 to M19-29, or whether it was like that all along, but it was definitely a crowded start.
Really need to get myself out in front of the slower swimmers.

Jump onto bike with shoes already clipped in.

Comments:

The bike was going well until mile 45 when I noticed that my rear wheel was half flat. I stopped and filled it using the CO2 cartridge. Once I removed the pump the presta valve stem broke and the tire had now completely deflated. So I spent the next 20 minutes replacing the tube.
What would you do differently?:

Bring a couple more CO2 cartridges and 1 more tube.
Need to work on more hill training.

Sorry to hear about the flat. I had the same thing happen right after the backside water station. Something got caught in my tire and I had to change tubes. Then again right after the last water station. I ended up waiting for the SAG wagon to bring me a tube, but finally started walking because I'd waited at least 20 minutes and figured I might as well. Luckily someone gave me a tube and the use of a pump, so I was able to get going the last couple miles to transition and rocked the run.

Those damn flats I tell ya. Argh!!
Despite your bike issues, it looks like you had a great race. Those are some impressive swim and run times. Congrats.

I had a lot of racers ask if everything was okay or if I needed help, while on the side of the road, and even had a cyclist, just doing her Sunday ride, stop to see if I could use any help. I really love the triathlon community.
