Subject: Custom training plans
Just took advantage of the BT 10 year anniversary offer and got a gold membership. Then I set about making a custom training plan. They're astonishingly brilliant. Just posting here in case there are people who haven't explored the wider functionality of this site like I hadn't done so much. I'm gobsmacked.

I'm considering upgrading and building a custom plan but I am unsure if it works for me. Can you build a Couch to Olympic plan here or is it mostly beginner and up?

You can play around with it now to generate free sprint and oly plans (without details) but you can see the workout minutes.  The longer the plan is, assuming it's set at the 'beginner' level, the longer the buildup period.  So for example, a beginner, Oly, 28 week plan, the starting week's volume is:

Week#
Week# 1
2013-12-09
Base

Monday
650 Yd
12m

Tuesday
850 Yd
16m

Wednesday
900 Yd
21m

Thursday
2.6 Mi
10m
0.8 Mi
08m

Friday
3.7 Mi
14m
1 Mi
10m

Saturday
5.2 Mi
19m
1.3 Mi
13m

Sunday

Week Total
11.5 Mi
43m
3.1 Mi
30m
2400 Yd
49m

So as you can see, it starts nearly at the 'couch' level.  Another option is to focus a month on getting your swim up to 900 yards, then start the plan. Though if you really have 'zero' base, you may want to start one sport at time and bring in a new sport a month later instead of all-of-the-sudden doing all 3 at once, even if the minutes seem low.

My left leg is significantly shorter than my right by 7mm. For the bike I got an extra shim in my cleat, so I'm wondering if orthotics would be called for in my running shoes?

Using a high-volume schedule for many months is not the best tactic. Build speed and skill several months out, and then transition to a high-volume IM plan.

I am in the process of buying my first tri bike. Because of my size my fitter suggested buying a custom frame but then I have an infinite number of choices for components, wheels, aerobars, etc.

This plan is an advanced plan in terms of volume and intensity and appropriate for someone who is looking to go under 5:00 and as fast as 4:30. There isn’t a lot of fluff in the plan.
