Do bar-end shifters usually come with cables?

Also, is it possible to just buy the little plastic pieces that go over the levers?

Looking at trisports, some of them do and some don't. For example, http://trisports.com/sh10barendsh.html

I'm not sure about the plastic things over the shifters, but if I were you, i'd just use some black electric tape.

The bar-con shifter kits (Shimano) come with cables and housing. At least if you buy them new they do. As for buying the rubber caps for the levers, I'm not sure. I'm sure you can find them somewhere, but I have no idea where (I've seen teh Campy ones on ebay occasionally).

walk in to you LBS and go over to the repair guys.  Tell them you are looking for the caps for bar end shifters.  They should be able to look through their parts catalog and find the piece if they don't have it in their big bin of parts.
