Pre-race routine:

Normal cold cereal & banana. Arrived about 10 minutes after transition area opened up.
Event warmup:

Ran 1/2 mile and stretched prior to swim.

Comments:

Deep water start with one wave including all men, made for a lot of congestion. I didn't fare well, swallowed a lot of water, and had trouble relaxing my breathing. I breast stroked to catch my breath and try to burp up some of the air/h20,but I couldn't get comfortable. One swimmer needed rescuiing at about the 2/3rd's mark, I only know as I got run over by the rescue kayak responding to him.
What would you do differently?:

More open water swimming. Should have swam that morning in advance of the race just to relax. Should have stayed outside the fray.

Felt dizzy trying to put on my shoes. Every time I bent over I'd get a head rush. Probaby was due to sinus being congested. Struggled to put on my shirt over my wet skin.

Felt a right hamstring pain through the first 1/2 of the bike. It was a new pain and had me worried about the run afterwards. Tried to maintain 20 mph pace. Was a little tough on the 1st 1/2 as there seemed to be a slight incline and bit of a headwind. Fared much better on the final 3rd of the ride pushing a solid 23mph. Tried to keep my HR in the low to mid 150's to save some energy for the run.

Typical brick sensation - felt horrible and my calves were hurting bad on run start. I stopped and did some quick stretching about 1/2 mile into the run. Slowly, I got my legs and picked up my pace slightly. I had some energy left at the end of the run, so perhaps should have pushed harder earlier into the run even though I felt horrible.
What would you do differently?:

Maybe try to push through the discomfort earlier or at least, finish stronger. Put my HR monitor on a bigger display setting so I could monitor it more easily on the bike & run.

Very poorly managed event. Enthusiastic volunteers, but... Poorly marked transition area & course, very minimal post race food & activities.
