I really like these:

They are REALLY soft and don't hurt my ears unless I have them in crooked. Then I just adjust them. Because they are so soft they are very bendable and a swim cap fits over them comfortably.

I wore the silicon ear plugs for a few weeks, but they did not seal good enough for me.  I am using a orange type that CVS sells (just like the type pirategirl is showing) the are for anything I believe.  This may sound too simple, but maybe you should get a razor or somthing and cut the end of the so they are not pushed in or down by the cap.

I use Mack's Ear Seals. The plugs come with a string that can attach to the plugs but I never use it. I cannot swim without them.

I use the fluorescent children's plugs.  The reg. speedo/tyr kind are too big/unpliable for my ears.  The kids' ones fit perfectly.  I can still hear the swim coach but not super well--I take them out if she's talking for awhile.

I really like the Finis ear plugs. When you put them in they don't feel like they are sealing, but they keep the water out and are very comfortable. Also, I can still hear very well with them in.

I second the recommendation for Doc's Pro Plugs.  My ENT doc had me buy a pair to keep on hand in the event that I lose my custom molded plugs.  If you can afford it, go with custom plugs.  They are usually ~$75, but you get to keep the mold if you need replacements made.  I went from having chronic ear infections due to whitewater kayaking and swimming (1 infection lasted almost 7 months!) to not having had a single infection in almost a year.

I also got chronic ear infections when I started tris. I have found that if I use eardrops after every single swim, I don't get an infection. When I forget, then I get one.
