Pre-race routine:

Up early & out the door. Not much breakfast, just a GU and Gu2O before the start.

Utterly disastrous. Once again, I completely panicked not long into the swim. If not for the wetsuit, I probably would have called for a kayak. Still, I got through it, even if most of it was on my back, flailing and backstroking.
What would you do differently?:

I need either more OWS practice, or maybe hypnosis, or both. I overswam the distance several times in the past weeks, so I know I can cover the distance.

This took more time than I remember it. I must have been more sluggish than I realized coming out of the water. The suit came off relatively easily, and I don't remember having much difficulty getting socks on or anything.
What would you do differently?:

A few things: Learn to do the swim in such a way that my HR doesn't spike and stay there. Actually run through the transition area instead of doing a zombie walk (apparently). Maybe learn to ride without shoes, to eliminate the delay of putting on socks?

It definitely felt tougher than the times I've done it with the training group. I suspect that's at least partly due to starting it after the panic-stricken swim.

Good news this time, though. I was able to maintain better pedaling technique, as my calves didn't hurt noticeably during the bike.
What would you do differently?:

Leave the water with a more normal HR. I also hope to do my next tri on a much lighter bike (Cervelo P2SL instead of the Trek hybrid I'm on now). And of course, much more time in the saddle between now and then.

Just move quicker. There were people taking less than half this amount of time, and I was feeling pretty good by this time. I just need to focus more and move.

Run more, do more bricks, and practice the T2 transition more.

Warm down:

Lots of walking, plus recovery drink, then an iced coffee.

What limited your ability to perform faster:

Panic in the swim! That was a horrid start, both mentally and physically. If I could have started the bike feeling like I did after my last 650m swim in the pool, I'd have been much stronger.

Congrats! 

How did you like the race? I live nearby and go to the Y and considered it but doing IM next weekend so maybe next year.

Just for some perspective, his disastrous swim was nearly three minutes faster than my nondisastrous swim.

I think all three distances were wrong... it's about 13 miles for the bike and they cut the run short and then added only a small fraction back at the end. The swim looked nothing like 550 yards, too.

I'm not sure that I can strongly recommend this race. I did it because it was hosted by the Hockomock Y, and I trained with a group there. That said, it doesn't seem like they corrected some of the issues from the inaugural race last year (no water at all on the run), and may have introduced new ones (all ladies in one swim wave despite their large numbers).

To follow up on Chad's point, I now realize that although I finished in the middle of the pack, I wasn't offered any water, either. From what I gather, it was an even smaller turnout than the first year, too. To be honest, I'm not sure that I'm going to do this one next year. I am doing the Fantastic Nantasket tri on 9/21, but I think I'll check out the Max Performance races next year.
