Hey wise peeps on BT--

First off, I have never had Plantar F (won't bother trying to spell) and don't want it. I wear orthotics and have been doing so for over a year now with no problemos.
(i got them when I had some achilles pain but had had them years ago after bunion surgery on one foot but just didn't wear them then).

Anyhow... pain started in arch and has been only a few times. But yesterday after cycling, it burned and had to take off my shoe to massage. Then today when running, I felt some stabbing pain but it seemed to subside a bit after 10 mins.

The pain is in the foot that had the bunion surgery too. I know my toe is 'stiff' and I work on stretching it but it is what it is.

My shoes have about 250-300 miles on them. But figure they still have some life left in 'em.

So anyone else have this happen? Maybe my foot is crying about cumulative beatings with orthotics? Maybe my shoes need replacing? Maybe its time for new orthos??? Maybe just not run for a week? Waaaaah! I have a race in 2.5 weeks.

i usually replace my running shoes around 250-300 miles. Depending on your orthotics, it could also be time to replace those. i'd start with new shoes now. Hopefully it's that easy.

My orthotics are hard...figured they last forever provided the leather doesn't wear out.
Jeepers... how long do they last?!?!?!?

Ask the person who made them.

My podiatrist generally recommends new orthotics every year or two and more often if there's been injury or significant weight change.

PLEASE go to your orthopedist and have it checked out.

I was in for a HUGE surprise when I went to my orthopedist (not my podiatrist) who is the medical supervisor for the NYC triathlon. 

What was the first thing he told me?  Not to wear the orthotics when running!  Isn't that nuts?! (I thought so).  He also sent me for a computerized running assessment (free here in the big city) and the assessors put me in - oh my gosh - a neutral shoe.  Not one with all of the support I was aiming for!

Ever since I stopped wearing the orthotics when running and was put in different shoes -  I have been a happy camper.

(And yes I am still shocked that that was the answer)

I had Plantar fasciitis  - FYI.

Please see your doc. They know best.

Very interesting story, DBW.  I may have to try to find that gate analysis here.

I've been wearing custom orthotics for 3 years.  I got PF in Feb 08.  It never went away.  The podiatrist tweaked the orthotics and said that should cure me.  Months later and no real improvement.  

I found that my feet feel fine walking around home (in only socks).  Its when I am on my feet for awhile (in orthotics) that my PF gets worse.  So I tried doing a few things without the orthotics.  And while my foot would still get sore, it sure seemed better than it did with the orthotics in.  So I've been doing more and more without the orthotics.  I strongly suspect they're part of the problem.

hmmm the thing with orthotic is that they are HARD and a 'forced - immobile' support that would, at least in my mind, bang your arch with each pounding of your feet on the pavement and potentially causing undue wear and tear on your arches. At least with most arches that are in a given running shoe, they offer some cushioning and 'give' vs. hard orthos.
I may have to delve into this further...thanx peeps.

Don't forget that orthotics were originally custom-fitted for people with severe foot problems back-in-the-day.

You may want to go to a good running store and see if your shoes and orthotics (if you really need them) are fitted properly.

I have realllly high arches and have been using the Sole heat-modable orthotics.  They are kinda expensive so I went to a podiatrist and had him fit me for custom orthotics.  350$ later, the custom orthotics set me waaay back on some tarsal tunnel issues I was dealing with and had to go back to the Sole heat-modable ones.  I've switched back to a more neutral shoe (like someone else had success with) and that's helped immensely with the tarsal tunnel but I still need the orthotics.  When I run without them, I can feel the stabbing pain in my arch with every step.  With the Sole orthotics, I'm pain free.

So I think you have to try what's best for you.  I'm yet to have a good experience with a podiatrist (the whole reason I got tarsal tunnel was because the first podiatrist I went to put me in a boot because she miss-read my MRI and told me I had a tear in my achilles and wouldn't listen when I said the boot was causing me pain, which caused my arch to flatten and strained my FHL tendon).  So listen to your body and experiement with some different combinations.  Write down what combo you wore and how your pain was/how you recovered.

The sole orthotics last about 2-3 pairs of shoes but I'd recommend swapping out your shoes if they have 250-300 miles on them.  That's the one good thing I learned from the podiatrist is that once you start to feel new aches/pains when running, it's time to swap out shoes!

I'm so glad you posted about the Sole heat-moldable inserts....as I'm about ready to give up on my custom orthotics, too. I've had some issues ever since I started using them...and here it is 7 months and 3 adjustments later, and I'm still not a happy camper. I've been considering the Sole inserts for quite some time now, and after reading your post...I think I'll pull the trigger and order some. I'm also thinking about ordering a pair of the Sole Slim inserts, for my bike shoes. Do you (or anyone else out there) have any experience with the Sole Slim heat-moldable inserts? They are supposed to be very low-profile and great for cycling shoes.

I have been dealing with PF pain for years ... and in October, I had a partial tear of the PF and was on crutches for a while because I simply could not bear to put any weight on it. After custom orthotics not helping, I switched docs and my new doc said to ditch the orthotics. And to use a neutral shoe. My PF pain now is mostly gone, for the first time in years, but, unfortunately, I've continued to have minor Achilles issues.

Have you been checked for Morton's Neuroma? I read one source that says 1 in 4 people have it, and to fix it simply cut a quarter-size piece of moleskin and put it on your insole under your big toe metatarsal ONLY and not any of the other toes.

You all have me very concerned! I was fitted for custom orthotics and they are here and I am breaking them in now. My PT said I needed them to correct a leg length imbalance on my left side, and an arch support issue on my right - my arch isn't the problem, but rather the ankle rolls, in order to level the hips out. Strength training will help some, but she said the ligaments are stretched out now, and that will never fix itself. Otherwise, I have high, stiff arches, and wear a neutral shoe.

Do you think I'm going to have problems now I NEED the lift in the shoe, and that can only be fixed by a custom insert (since it's minor, but enough to cause problems) - I've been running with a temporary lift, which helps a lot, but it's not made for long term use. And my right foot still overpronates. Hmm...I'm worried

Be sure you break them in on schedule. I think an hour a day, right? If they are fitted correctly, repeat if they are fitted correctly, you'll be fine. Be sure they are fitted and get them adjusted if anything feels off. You know your feet better than anyone.

Yes, an hour a day until I can build up to a "day" (like 8 hours or so) - she said that would take about 2 weeks or so. Then running in them 20 minutes at a time, until I can go longer, etc.

This is actually the second pair, as the first pair was from a different company and they were horrible - my feet fell asleep after like 20 minutes and they were so big, I had to buy a new pair of shoes, a whole size up, and then the toe box didn't fit right, etc. But now my PT is on her own, and can choose the supplier she likes. I've actually been sitting around in them and barely notice that they're there. Just concerned about what will happen if I start running. I don't want my left arch, which doesn't need any support to be angry

You should be fine. It should be possible to build your orthotics the way your body needs them. If they aren't perfect, let your PT know.

For what it's worth, my feet are kind of a disaster, and i've never had problems with orthotics.

I'm pondering getting "soft" custom orthodics.  I did have hard orthodics 7 years ago (correct for leg length and arch issues / ITBS) but they were horrific and I tossed 'em.  But I'm seeing a new sports chiro who says the "soft" orthodics are great for runners. 

I currently have motion control shoes (that I like), but won't I have to switch to a neutral shoe with the orthodics?

Not all orthotics are created equal...you really need to get thee to a sports podiatrist. Also, an orthotic that is used for running will be different than one that is used for everyday wear. If you have serious issues and need to wear them all the time, your doc will create an everyday orthotic and a running orthotic for you. Are you sure you're supposed to be running in the orthotic you have? A running orthotic will need to be replaced every 2ish years. You should be in a neutral shoe with a custom orthotic. When you stand on the orthotics (even just barefoot when they're not in the shoe), it should give you near perfect leg alignment. You don't need any help from your shoe at that point.
