Hey all!

I have a question and mostly I know that I should go in and see someone - but I don't want the answer if it's bad ... and otherwise I don't know what I should be expecting...

I fell about 4 weeks back and smashed my shin off my pedal.  It swelled up pretty good and I still have quite a "bump" under the skin right on the bone.   This however doesn't hurt much when I press on it.

I also have problems with shin splints and I haven't been running with my orthodics because they've been bothering me...

So now I'm to the point where it aches all the time.  Sometimes it's just tingly and sometimes it really hurts.  I haven't run since last Saturday because I'm kinda worried.  If I push on the inside of my tibia about half-way between my ankle and my knee it hurts, but it doesn't hurt when I stretch it or push anywhere else.  It does hurt to run - but it goes away.

I know ... go in and have an x-ray and bone scan....

I had a tibial stress fracture and pain did NOT go away when I ran, it got worse. It didn't really hurt at all unless I ran or walked or pushed in on the area really hard. The pain was VERY localized. Someone asked me 'what does it feel like', and I said 'it feels like my leg is broken'. That was what finally sent me in for a bone scan. Can't speak much to the other issues, but pain getting better with use sounds better than the opposite.

How about RICE, does that help? (rest, ice, compression, elevation I think it stands for). Massage? Acupuncture?

Because you have had to get orthotics in the past and are bugged by shin splints, this sounds like a good opportunity to go see a sports doc/PT and get it all checked out. I agree with annabanana, a fracture/similar would probably hurt more with activity, you might just have some bruising/swelling. You are smart to rest, RICE it and if it were me, I'd want a doc's opinion.

I totally agree with all of the above comments. As a sport-specific physical therapist, I have seen several stress fractures, episodes of shin splints, and compartment syndrome. A stress fracture has symptoms that are localized to one area and increase with activity. Chronic compartment syndrome has symptoms that increase with activity and is painful with passive stretching. Shin splints can be very annoying. The underlying biomechanics need addressed to prevent further damage. If the orthotics are uncomfortable, they need to be revised or re-made. When I make orthotics for clients, it may take up to 2 months to get them just right and they must be comfortable.Many other biomechanical errors, regions of muscle weakness, and tightness of certain muscles can contribute to shin splints. It would wise to consult a specialist as soon as you can so you can enjoy your season. The specialist could also rule out any other contributing factors, such as an underlying fracture. I would start with an orthopedic physician that specializes in sports and the lower extremities. I wish you the best.
