Depends on how you wear it.  My swimcap NEVER goes over my ears completely.  Its always pretty much the top inch tucked in and earholes uncovered.

I won't wear a neoprene cap for the same reason.  And the chinstrap makes the constricted/claustrophobic feeling worse.

I wore 2 swimcaps for Boise 70.3 last year (52-54 degree water depending on who you talk to).  One latex under the issued IM one for warmth.  I was absolutely fine.  HOWEVER I did stumble a bit going into transition.  I'm sure this was due to the cold water in my ears.  I'll take that over the claustrophobic feeling any day.

This isn't really answering your original question, but you are better off just wearing two swim caps, one of them silicone.  That neoprene cap will chafe the daylights out of you...

I put on the neoprene cap then the event cap. Then I just pulled the chin strap up and over my head to the back. I don't like the chin strap either. I've never had any problems wearing it like this.

No cap is going to cause you to swim in circles if you put it over your ears, but different types of caps may fit differently. Caps, in general, help to keep your warmer and avoid heat loss, but when I swim in cold water I just wear 2 latex caps. The thicker the cap, the more likely it is to fall off and slide around. You do not have to cover your ears when wearing a swim cap, I don't and personally hate to.

Ear plugs also help to keep warmth in and is recommended for swims in very cold water. I think the Alcatraz swim is 1.5 miles in 65 degree water, with a lot of waves and currents. They try to time the swim with the currents and for this race I would highly recommend cold water acclimation and wearing two caps and ear plugs.

Cold water can be disorienting if you are not use to swimming in it for long periods of time, so be careful and good luck!

I never cover my ears with the swim cap.

Ever.

No cold water issues.

I regularly swim during the year in water that's high 50s and low 60s. 

My feet are another story. I can barely walk getting out ofthe water.

I spin squares for a good 20 minutesfollowing a cold water swim.

Swim in cold water more?...
