Pro 15655 | Subject: Ibuprofen gel I was going to post this on COJ, but in light of the doping thread I figured here'd be good enough. Do any of you use 5% Ibuprofen gel that is available over the counter in Europe and Canada? If you take motrin, or any of the other brands here, and have not tried the topical gels available overseas, you are missing the boat. I'm not a doctor....but if Ibuprofen helps you...this is a much better alternative as it is not ingested through your stomach. Do an on line search of Boots Ibuprofen gel and you can order your own. I have some fairly substantial arthritis in my hip......this gel takes the pain away. Obviously, it doesn't help the arthritis and I'm not delusional (OK, that may not be as obvious) enough to think it does, but I gain much more pain relief from this topical application than I ever do from the pills since it is applied right at the problem area....with ZERO stomach issues. (Think Alieve vs. Advil) If you've got a reasonable doctor, the Rx shouldn't be hard to get if you think it might help with your aches and pains. I was given it for my IT band problems and it seems to help my flare-ups as much as taking naproxen orally would. |

At some race I did a few years ago I got a goody-bag that contained a spray-on analgesic / anti-inflammatory that contained aspirin (or a derivative) and some other ingredients (menthol, camphor, etc.). It helped with local aches and pains. |

Just like most other topical patches and many creams/gel, it has no "local" effect. Doesn't matter if you rub it on sore joints or your forehead. And it's not any safer from GI toxicity either...only an upset stomach if the tabs bother you, but the gel carries the same serious risk of ulceration/GI bleed as the pills (albeit quite low with occasional use). (ps, when I'm not training I play pharmacist) -MIke |

Have you tried buying Voltaren over the web? Regulation issue. Costs them money to get FDA approved for OTC. If they figure the sales will not be worth the hassle, they won't do it.

I appreciate what you are saying with your experience. In my experience using it, there is NO COMPARISON to how helpful it is topically on my hip as compared to taken orally. BUT.....I'm sorry, those pills being absorbed through my stomach vs. the gel being absorbed through my hip.....you're killing me. I respect everyone who has an opinion on something in their field...it makes me sit up and think about what they are saying.....I can't get behind what you are saying with regard to GI issues. My experience doesn't come close to bearing out what you say in that regard, and I've got a cast iron stomach. |

(ps, when I'm not training I play pharmacist) -MIke I wonder, though, if by applying the medication topically at the location of the pain/inflammation, more of the medication might effectively reach and treat said pain or inflammation? I understand what you're saying about the medication being absorbed through the skin, but it seems maybe it would have less distance to "travel" to the point of pain if applied topically than if taken orally. Of course, I'm no pharmacist, and I may be completely wrong in that assumption.... just curious!! |

Prostaglandin synthesis is responisble for the protective lining of the stomach which prevents the extremely low pH of the stomach to damage the lining and cause bleeding internally in the GI system. Prostaglandins are also one of the components of the inflamatory response which is why ibuprofen or naproxen inhibit it. So it may not upset your stomach the way a multi-vitamin can because of the route it is absorbed, but it can still cause ulcers and GI bleeding. Darin (PA when I am not training) |

I don't doubt that there is some relation to the topical medicine and the stomach lining, but it can't be as bad as oral NSAIDs which can occasionally cause a perforated ulcer (if not taken with food) resulting in emergency surgery. I am not a medical doctor, but this happened to a family member who has now been banned from oral NSAIDs. If you are taking oral NSAIDs, it is important to eat and drink plenty with them. |

That would determine how well it works as well as how well (good or bad) it negates prostaglandin synthesis. The key is the same for both use sensibly and not too much. Your family member may have been sensitive to the drug as some are and it worked exceedingly well (good or bad) on their prostaglandin synthesis. I really like to say prostaglandin synthesis. |
