I've become much better at avoiding my chain coming off when switching from big to small ring but it still happens from time to time.  Two questions:  How do people avoid this?  Is there anyway to get the chain back on without stopping?

1) don't be in the smallest or largest cogs on the rear when you shift to the other ring up front. If I'm in the bit cog on the rear and shift to small up front, I lose the chain. On my 10 speed rear cassette if I'm in 4, 5, 6, 7, 8 I'm okay going to/from either of the front crank.

2 If it's inbetween the two cogs in front and just not caught on either, you can finesse it back to the large cog by peddling slowly and sneaking the friction shifter from small to big.
If it's inside your small cog up front you're pretty much screwed.

Front derailleur alignment, front derailleur set screw adjustment.  Even if you are crossed up, it should not throw the chain if it is set up correctly.

UNLESS ........ you have a Shimano triple front.  Then it is really tricky to set up and often hard to avoid .....

Yeah but when you got a new bike and the cables are stretching almost daily it happens until they quit

FD alignment and/or limit screws are not affected by cable stretch..  Get to your LBS and have them turn the limit screw down a smidge.

x2 on the friction shift fix for putting the chain back on.  You should (almost) NEVER have to touch your chain with your fingers.

Exactly.

To put it on (unless you are on a hill), all you do is soft pedal and shift back up into the big ring, then when it is back on, shift back down to the small and that way you do not have to stop riding.  Very easy to do.
