Many doctors do their hospital rounds before coming to their office. The self-employed doctor who sets his own schedule is largely a relic of the past. Most doctor's offices are owned and managed by a medical practice company. The doctors in the practice may or may not be partners. Either way, the management company sets the scheduling of consultations based on insurance re-imbursement rates for office visit and desired income level that the practice wants to achieve for each doctor on staff. Determined by the insurance rate and the desired income, the doctor is given a small amount of time to meet with patients and move on. It is hard to keep up the pace all day long.

Because when they walk in the door there are 10-15 emails (most of ridiculous nature) and VM that need to be returned and some require them (not the nurse or the lab manager) to do some immediate paperwork, if a patient needs to be admitted or seen ASAP.
