any insight on why my right knee gets sore (enough to have to slow my pace WAY down or even stop within minutes) each time I run now - and it begins always between 14-17 minutes into my run (starts out fine).  I am going to take a short break from running and HOPE that will be enough as I have a sprint in less than 2 weeks.

short history - I USED to have troubles with BOTH knees when I first tried to run a couple of years ago when I was WAY heavier.  I was diagnosed with Patello Femoral syndrome - got orthotics (which I still wear religiously)....the orthotics and weight loss seemed to do the trick - My knees have been mostly pain free for about 1.5 years now.  The pain I am having now is on the outside part of my knee cap and below.  I am wondering if it might be something as simple as getting new orthotics (since mine are over 2 years old now and I got them when I weighed a lot more)....what is the lifespan of orthotics anyway?  Or maybe it's my new runners - but I have had them for nearly 2 months now and they didn't bother knee initially?

I will be seeking professional help if a week off of running doesn't cure it...but would love any insight you might be able to offer in the meantime - thanks.

forgot to add that the orthotics were to correct over-pronating (four pregnancies really stretched the ligaments in my feet and they got really flat)....as well - it was discovered that I have a leg-length discrepancy - so a slight lift was added to one of my orthotics.

i'm not a doctor....but i do play one on tv.....

if the pain is on the outside of the knee, it could be a IT band issue.

Not a doctor, but I'm a nurse!! Which is only pertinent because I'm a nurse who happens to have had similar issues. In my case it was ITB . Same story as what you describe:happens 15 or 20 min into the run. I got some wicked good ART and also changed my running terrain. I was running on roads which have a slope and even though I switched sides frequently I still ended up injured.

Between the ART and terrain change my issues have been resolved. I keep my IT in shape now with a foam roller.

If I may add one thing, you mentioned that you haven't changed a piece of running equipment in over 2 years!!! I must say that has to be part of the problem....

thanks for the info - it DOES look like ITB syndrome (once i looked it up!)

that doesn't look good for the tri I had planned on the 27th!  I read that I need to cut out/back/restrict running and cycling and then slowly wean back into it....

I will be calling tomorrow to get a formal assessment/diagnosis....I am not really sure what is causing this - I don't *think* it is my training schedule because I didn't really do anything recently that I hadn't done before - I was actually running LESS lately than I had earlier in the year.

I read that overpronation, leg length discrepancy, wrong shoes can cause it - and if my orthotics are no longer working as they should then they might very well be the culprit.

I am so BUMMED!!

I am glad you are getting an assessment done. I would go with the orthotics being the primary reason, plus the leg-length. Get a massage and some new orthotics and I would think you might be feeling better. I am off to the bike today.

thanks.... I've got an appt today at 2:00 - with a chiropractor who works out of my personal training gym.  He also does orthotics, ART, and running gait analysis.  I think he is a good 'first line' person to see - and then go from there.

He is also an experienced runner himself (runs sub 3h marathons).  And he will be able to tell me what/how to alter my personal training sessions because he is familiar with all of the trainers and what they do is a bonus too I think.

Lets hope it's an 'EASY FIX'....and that I still have some training options....

When was the last time you replaced your shoes?  Just a thought.  At about 6 months with one pair of shoes my knees start to hurt.  New shoes and the problem is eliminated immediatley.

I know Chris - i am so bummed! I am so stubborn that I am still 'hoping' and crossing my fingers that there may be some way I will get the 'approval' to still do it (and just not 'race' it)....but part of me doesn't want to do it if I CAN'T 'race it', ykwim?

Matt - My shoes aren't that old - I just bought them at the end of May (on the 28th - I just went and looked up my receipt to be sure).  I ran in them all of June with no problems so I don't think it is the shoes....

If it is IT band syndrome, one thing that comes highly recommended is the foam roller-very cheap fix that has helped tons of runners. Good luck with your visit to the Doc.

Foam rollers are great for your IT Band, actually great for all muscles.  I highly recommend it if you haven't tried them before.

Hey sorry to hear about the ITBS. Glad to hear you are going to see a doctor/chiropractor who can prescribe some stretch/strength exercises into you regimine.

I got ITBS over a year ago, and stubbornly decided to not address the issue for 8 painful months. Over the past several months I visited a PT and had a gait analysis done. Both were equally important but definently think the gait analysis was more critical as the physician noticed that I "overstrided" which put unneccessary pressure on my IT Bands. Since visiting with the doctors I have been slowly increasing my runs and yesterday ran for 30 minutes straight for the first time since April.

Best of Luck and let us know what the chiropractor says.
