The park or locality could fine you or remove you from the park. There may be a USAT rule or a race specific rule I don't know about. I personally pee on the run (not races ) often. Pick a wooded area and make sure no one is watching. Yes, when I come out of the woods and someone sees me there is not much mystery as to what I was doing but on the running path it is understood by most if not all others (as long as you didn't litter or expose yourself ). During marathons I have peed in discrete locations every time. Assuming no-one sees your nakedness or act of peeing (even if it is obvious what you were doing) : Is it a penalty - prob not. If it is, is it worth it? If you gotta go, you gotta go. Is it bad manners? No. If some one sees you nekkid or going - penalty - yes, is it worth it? better to pee yourself than do it in view bad manners - yes
