I always go to Express or The Limited. They make dress shirts that are fitted and some have stretchy fabric so they don't blouse out.

This.  I always have to buy the shirt to fit shoulders/arms then get it tapered for the rest of my torso.

And I totally disagree with ANN TAYLOR.  They somehow think every woman has Barbie doll arms!!! 

I joke my genes were the farmers that survived the Irish potato famine so they needed strong arms to plow whatever fields were there, but honestly not every woman has arms like twigs!

as I was transitioning out of the military we have to go through a transition course and one of the things they do (or at least the one I attended) is have a woman come in who is a professional image consultant/personal shopper and she spent time talking about buying off the rack, getting stuff tailored etc

she said even if something fits (in your perception), take it in and see what a professional tailor can do - because even a tiny nip and tuck can make something look 10x better - for example, with a straight A-line skirt...she says ask the tailor to narrow it down about an inch (1/2 inch on each side) because it causes a slimming effect and makes your legs look longer

I have to get blouses etc tailors - thank you broad shoulders/big boobs; but like others, I find knit tops work. I also tend to favor dresses and just do a nip/tuck at the waist

Also, sometimes you can wear a camisole under a blouse and leave the first 3-4 buttons unbuttoned if the bust is the only problem area.

I think part of it, is the related threads box that pops up underneath discussions...there have been a few times I have seen one that looks interesting and when i've gone to comment, i've realized that it is a thread from 2008/2009 etc

Exactly.  Leave it to the guys to come to a thread about Women's Dress Shirts (probably hoping for pics) and offer the simple fix of sewing a dart or two into the shirts.

If you happen to be in Hong Kong (on business or pleasure, whatever) get a bunch of tailored shirts. You get to choose your fabric and style, they measure you, and you will get a very well tailored shirt (or you might as well go for the full shirt/jacket/skirt/pant combo) in 24-48 hours. Hong Kong has some of the best tailors around.

Obvs it's ridiculous to go to Hong Kong to buy these things alone, but if you happen to be there...besides, it's not too far for the Aussies and Asians here on BT.
