Yesterday after a run (treadmill), I developed intense pain on my left illiac crest....the boney protusion at the top of the pelvis. It's sore and tender to the touch, right on the bone. There doesn't appear to be any bruising, but it feels just like what a bruise would feel like. I had to skip my run today, because it was painful enough just from walking. I am 3 weeks out from my first sprint triathlon...which makes matters worse. I have recently done some longer bike rides - I did 75 miles on Sunday, and 36 miles on Monday...so over a 100 miles in 2 days. Could the increase in my cycling be a contributing factor here? What is IT Band syndrome? Could this possibly be the same thing? Any help or info would be greatly appreciated!!!!

Not a single reply to this? Hard to believe that nobody else out there has this type of hip pain or IT band syndrome? There has got to be somebody out there with some insight...anyone???

There are a couple of threads going around about others with similar pain.  I have the same pain that you are describing but have yet to go to a doctor to get diagnosed because I am self employed and being cheap.  I just started seeing a chiropractor and x-ray did show my hips are misaligned.  If you find out anything specific please share it because this is an annoying injury.

It sounds like the pain is on the bone itself? What type of pain is it -- aching, stabbing, sharp, etc, etc?

ITB Syndrome (also known as the Devil) is a very common cause of lateral knee pain for runners and cyclists. The Illiotibial band controls the rotation of your knee as it moves throughout a stride. In almost every manifestation of it, it has been knee pain, not hip pain.

However, have you considered and injury of the hip flexor? I tore mine during sophomore year and it wrecked my season. It manifested as pain on the top of my hip, just below the crest on the top of my thigh.

Check with a doctor. If your insurance is down with it, you might save a week by going straight to an orthopedic.

OK I have had a similar issue. Top of the iliac crest was prime pain spot, but it did move around - glute medius, hip flexor. Had an MRI but never a diagnosis. I am back to regular exercise. I *think* it was overuse when I was running quite a bit and then enthusiastically added in biking on a different bike (road bike after riding hybrid). PT said it was psoas, and I do stretches.

The other thing I have heard of w/pain in that area is hip bursitis.

Google "hip overuse" and see what you can find. Good luck!

Hi, thanks for your insight. Yes, I've googled hip pain...and have come to the conclusion that it's definitely NOT hip bursitis, as bursitis originates on the ball/socket hip joint (trochanter). My pain is much higher like yours...iliac crest, and yes...sometimes it extends to the glute medius and flexor. I'm fairly certain it was an overuse issue with me, too. I had really ramped up my cycling - had gone from doing 25-40 mile rides to 60-75 mile rides...that's a substantial jump in mileage in a short time (2 weeks). I've been doing tons of stretches geared for IT band syndrome, and they seem to be helping somewhat. I'm curious...are the psoas stretches you are doing similar to IT band stretches?

here is one thing it could be. You could have knocked your pelvis out of alignment. The illium can rotate forward, backward, flare in/out, and move up / down. It can cause pain at the SI joint, glute pain, and pain in the groin and hip flexor. It is one possibility that could have happened. You would need to see a Physical therapist that has some good manual skills to get checked out.

Linda - no the stretches are not IT band stretches. Craig had good advice about seeing a good PT. If you want, PM me and I can try to describe the stretches. Keep in mind, though, that I am unusually flexible and do not need IT band, hamstring, etc stretches so your case may well call for some other treatment than what I had.

Yeah, in retrospect, I feel pretty sure the ramped up bike mileage plus change in equipment from hybrid bike w/sneakers & cages to road bike w/clipless pedals caused this problem for me. Good news is, rest and a gradual return to biking and running has me just about asa good as new.

Hi Linda,

I have had the same injury for ten months but don't know what it is and haven't been able to walk long distance or run.  Last summer, I had increased my running and cycling substantially and started to run sprints and hills.  After 30 years of running and minimal injuries, I felt pain on my iliac crest in mid-Sept.  It also was tender to the touch, right on the bone.  After 10 days, it hurt just to walk and I went to an Orthopedic doc and phys therapist who both said it was a glute tear.  Three months later and two more docs, two more therapists, and 2 MRIs, it was discovered that I had a labral tear and had surgery in January.  Unfortunately, the iliac crest pain persists.  Currently, I am going to a therapist who does Active Release.  She thinks it is tight muscles, tendons and the IT Band tightening around the crest causing the pain and mild bruising.  The first two treatments have left it more painful but I will continue with it since no one else seems to know what it is or what I should do to resolve it. 

Are you improving and able to run?  If you discover what it is and what to do about it, please let me know.

Sorry to hear that you are plagued with this issue, too. I'm happy to report, that for now...the pain on my iliac crest has subsided. I had been doing some of the IT band stretches that were suggested to me from this forum. I was skeptical at first, but I guess they must have helped because the pain has disappeared and has been gone for about 1 month now. But I won't be holding my breath that it won't return at some point. I usually get a flare-up, once or twice a year. I still don't know if the cause of the pain was from running, from the increased mileage I was doing on the bike, a combination of both, or something else entirely. It's frustrating for sure, and I do hope that you're able to get to the bottom of it soon.

I have this exact same pain.  Started about 3 weeks ago when I was running some hills in Oregon on vacation.  There are no hills where I train in south Texas.  Have laid off it for several days and have been icing every night.  I have continued to swim and cycle as those activities do not seem to bother it.  It is definitely a weight bearing issue.  Most painful when heel first hits the ground.  By the time I am pushing off the ball of my foot it is no longer painful. 

Current thought from PT and ortho guy is quadracep insertion tear on the contracting muscle.  I think I got that right.  Supposed to walk little, run none and keep up with the ice and Advil. 

 

I suspect mine was caused by the increase running times, hills and distance all in the same week.

 

Hope yours continues to heal

I have the same symtoms and it has been diagnosed as ITBS. It definitly is more common for ITBS to affect the knee but not unheard of for the pain to be in the hip since he band runs from the hip to the knee.
I went to PT and continue to do the IT stretches which seem to help.

I had this last spring. There was an article in triathlete mag (aug issue i think) that described it as illio-psoas strain. I did psoas stretches for it and it pretty much healed. Also started doing a lot of core strengthening, which helps.

The article indicated that, while painful and annoying, it doesn't tend to be a serious injury. The only problem is that, if you're in pain, you will change your gait, which can lead to more serious stuff.

hey all, this Iliac crest pain seems to have a few different sources of pain. The most common complaint I get from patients is a palpation pain right underneath the anterior superior iliac spine ASIS- its usually an inflamation of the insertion of rectus femoris (one of the quadriceps). It does sound like a few of you may have the beginnings of this.

If the pain is 1-2cm medial (closer to your midline), its likely a hip flexor (either ilopsoas or iliacus), usually right in the attachment to the inside of the iliac crest.

 If the pain is 1-2cm lateral to the ASIS, its usually a glute/ ITB pain.

 

Try to differentiate between inflamation and contact pain..... google inflammatory symptoms, as well as muscle strain symptoms, and it should give you (and me) a better idea of which structures are involved....

Iliac crest is a common place for hip pointer injuries - from separation of the muscle from the bone to full out avulsion of a piece of bone. 

So if pain is persistent you NEED to see a doctor!

I have had a similar pain on the outside of my right iliac for a year now. Not sure exactly how it started or a specific day, just noticed the pain the one day when walking.

 

At that point, I was running 5.5 miles three times a week on a treadmill. It felt, and still feels, like a sore muscle. After extra stretching did not help it, I stopped running and just walked the same distance. The pain reduced slightly probably because I was not banging on it as hard. After a month of walking and still the pain, I did a month of yoga and no impact exercise. Maybe a slight decrease in pain. Next was PT for a couple months. This helped only a little. I seemed to get to a point and plateau. PT included stretching, strength exercises, ultra sound, electrical stimulation, and somthing calles ASTYM to help break down the scar tissue.

 

Off to the orthopedist where a I got a cortisone shot. The next day was great. The shot was done at the center of my pain and it was all gone but for a couple surrounding spots. Unfortunately, the pain slowly came back and I was back to full pain in two weeks. Once I could get another cortisone shot, I started PT again with the pain gone to see if it needed this kick start. Pain back in about 10 days.

 

Now I am seeing a pain specialist and so far no luck. Tried intra muscular stimulation and nothing. A shot of some other med around the crest following the nerves and nothing.

 

X-rays were clean. MRI of the spine was good.

 

I feel no pain when getting palpated. I feel it when I cross my legs and stretch to the side and front. I do feel bits of pain here and there with other everyday movements, but not always. The pain is not stopping me from anything as it is not hellish. I can press on the area and make it hurt, but I have to dig in. This tells me that it is deeper in there. It is located right under my waistband on my pants, maybe a couple centimeters below the top of the crest.

 

I have done a lot of reading and it seems likely that it is something with my iliopsoas muscles, but don't know. I am still doing all the stretching and exercising from the PT. Also, I took a month off with no impact exercising and it did not improve, something that helped others. I did get my gait analyzed. It is not perfect, but nothing that would cause any issues. I don't know if the pain is causing bad form when running or bad form is causing pain. I don't really feel it when I run, sometimes yes, sometimes no.

 

I have been a runner my whole life and nothing like this. I know that this pain can cause me to have bad form and lead me to making my left side hurt a little. But that will go away. The right never improves. Maybe something is out of position and constantly aggravating the area, but no one sees anything of relevance.

 

Appreciate any input.

I had a pain like this for months once during my tri career.  I had to withdraw from IM California (does that tell you how long ago?).  I went to about 6 doctors.  One of them told me it was a female prob - go to the gyno.  Gee, thanks.

My pain was DEFINITELY on the iliac crest and later included pain in the internal/external obliques.  I had an xray, 2 cortisone shots, 2 months of PT, iced, stopped running, still no real help.  It hurt to twist to put on my seat belt!  I thought I was finished.

Finally, after much research, I began to believe I had a leg length difference.  I had been running 1 time per week on the track (speedwork).  Running on a track or cambered road can cause a shortening of muscles on one side resulting in a leg length difference.  (Although not usually a treadmill)  I went to a renouned chiroprator here in Atlanta (helps all the big time marathoners and was a doc for the olympians).  He confirmed it - it was a leg length difference.  After about 6 visits to the chiro I was ALMOST pain free and able to complete an IM later that year.

Now I stretch religiously and get adjusted 1-2 times per month.  A stretch that helps is to do a straddle stretch while sitting on the ground.  Then, take your right arm over your head and reach to the left foot.  Don't let your chest fall forward.  Repeat on the other side.  Now I can easily reach both feet with my hands without my chest falling forward.  This is also great for the hamstrings!

Having said all that, my experience may not be what you are experiencing.  I just offer what happened to me in case it may help you.

GOOD LUCK!  I see you posted about this in June.  I hope you are pain free by now.

not sure how I missed this one sorry.  I actually have had the same thing at varying times on both sides.  Its weird.  Its most likely Iliotibial band syndrome.  The ITBS originates on the ilium (hence ILIO tibial band).  Certainly a malaligned pelvis could contribute.  SOunds like you are better by now tho.  For those of you with long standing problems I would keep searching for a good sports med doc ad be sure to rule out a stress fracture.  http://www.sportsmed.org

I went to a PT today and she told me it was psoas too! She didn't think it was psoas at first though.

I have been "Jogging" for 10yrs. When in Scotland running round flat parks or tracks I was fine. I then moved to Sheffield and ran up and down hills. This caused me to have a tender Illiac Crest area which as you say -  resulted in my hip bone being tender to touch and pain when I walked after sitting for long periods of time.  My GP referred to me to a physiotherapist who over 6wks made me do excercises to re train the muscles (strangely enough they were in my buttock area). This worked - and for the last 2 yrs I have not suffered from it....until now! I have started to do the excercises again but I can only remember a couple of them... its too difficult to tell you how to do them but I am hoping someone in my office ( I work for a private healtcare company) can email me a description. If I do get them  - I could forward them to you to try...but my advice is to see a physio.. she really worked wonders for me.

Sounds like a gluteus medius muscle strain. Rest, ice, stretch, lower mileage and stop when you get pain. This muscle keeps your hips level when running so an aggressive increase in mileage can easily cause this.

It is not IT band syndrome or hip bursitis.

This happened to me after a particularly strenuous bike brick. I found the piriformis pigeon stretch to be the ONLY one to relieve it. I'm going to a PT this week to see what I can do to strengthen whatever needs strengthening. It just killed any thought of running!

I'm experiencing much of the same thing and have learned it's primarliy due to misalignment and overuse.  This caused muscular imbalances so I've had to slow down and start all over by focusing on core strength, quads/IT band, glutes combined with hip flexor and psoas stretching exercises.  I'll continue this process and will see my PT once every two weeks until all the pain is gone.  I'm guessing this will take a few months.  Recovery time will be a focus when I start my Ironman training again and I'll pay very close attention to the little things along the way that I just allowed to fester before.  I'm still biking and running but very light and short mileage, no heavy/fast intervals or long, long rides/runs.  For now, I'm missing my bike/run intervals and long miles but this down time is allowing me to recover and direct more energy toward technique/education.  Besides, I needed to work on my swim stroke and conditioning anyway.

ok, so after searching for around 2 months this has to be the closest thread I've come across that refers to my issue. So, a little history. I'm not a runner, but I do play indoor football 3 times per week for 2-3 hours each time. Around 6 months ago I was getting a dull ache on my left crest of iliac right across the top and along my abdomen (in line with belly button). What is also noticeable from the iliac is a stringy/fibrous thing about 7 - 10cm also running towards my belly button. This at times feels swollen - usually when everything else is tender.

I don't have stabbing pain. The area is sore to touch like a bruise and I'm pretty concerned about this stringy/ropey thing (I guess it's tendons or something) that is also sore to the touch.

I've had some serious injuries playing football over the years, left shoulder contusion, vertical left wrist fracture and complete left knee patella fracture. Not sure if any of these are contributors as I get no issues from these past injuries.

I did see an orthopedic doctor and said it's to do with kicking the football (over stretching, repetetive stress). But I've stopped playing for a while and it still flares up frequently.

Hope someone can help.
